If this is your first visit with us:

• Please bring a copy of your insurance and picture ID with you to our office.
• Please bring a copy of any referral if provided by an outside physician
• Please arrive approximately 15 minutes early if you have not already filled out registration and health questionnaire forms online.
• Please bring with you any completed imaging reports or images you may have. If not in your possession, we can obtain a copy of any previous reports for you.

 

If this is your first visit with physical therapy:

• Please dress comfortably for your initial appointment. We will provide gym shorts if needed.
• Please bring a copy of any therapy prescription if provided by an outside physician

 

If you have an electromyography (EMG) study scheduled:

• Please allocate approximately 45 minutes to complete testing
• Avoid use of moisturizers, lotions or any other skin creams the day of testing
• Please dress comfortably. We will provide gym shorts or a gown if needed
